The RCS School Attestation Form must be filled out before your child can return
to in-person learning when Plan B starts on September 21, 2020. Thank you for printing and completing this form before your child returns to school. Kindergarten through 3rd grade families will have an opportunity to fill out the form when their children come to school for their MClass Assessment. Forms are also available at school for any family who wants to come fill one out ahead of time.
